Coaching Engagement Agreement

Please review and accept our terms and conditions to proceed. Your agreement to these terms is required to use our services.

Service Agreement

Agreement between: Coaching Executive Functioning LLC (“Coach”) and Client whereby Coach agrees to provide Executive Function Coaching Services for Client focusing primarily on Client’s stated goals, including but not limited to: personal and professional development, enhanced communication, organization, problem-solving, and independence in activities of daily living.


Description: Executive Function Coaching is a collaborative relationship between the Coach and the Client in a thought-provoking and creative one-on-one process that inspires the Client to maximize their personal potential.


Services: Clients agree to participate in Executive Function Coaching

  • Each session will be approximately 50 minutes, conducted via videoconference or telephone.

  • Clients will determine the frequency of the sessions. Sessions will be scheduled by the client subject to Coach’s availability.

  • Clients will have access to the Coach for additional support via email in between scheduled coaching sessions on an as-needed basis.


Fees and Payment:

  • Coaching fees vary depending on the coach and service option selected. Monthly coaching packages and individual 50-minute sessions are available. Current rates for all services are posted on our website, and specific fees will be discussed and confirmed prior to beginning services.


Responsibilities:

  • Coach agrees to respond to Client’s emails on weekdays within 24-48 hours and to advise Client in advance of any times of extended unavailability (vacation etc.).

  • Client is responsible for creating and implementing his/her own physical, mental and emotional well-being, decisions, choices, actions and results. As such, the Client agrees that the Coach is not and will not be liable for any actions or inaction, or for any direct or indirect result of any services provided by the Coach. Client understands coaching is not therapy, does not substitute for therapy if needed, and does not prevent, cure, or treat any mental disorder or medical disease.

  • Client understands that coaching is not to be used as a substitute for legal, mental health, medical or other qualified professionals and will seek independent professional guidance for such matters.

  • Client agrees to communicate honestly, to be open to questions, feedback and assistance; and agrees to create the time, space and energy to participate fully in the program.


Confidentiality:

  • This coaching relationship, as well as all information (documented or verbal) that the Client shares with the Coach as part of this relationship, is bound to confidentiality but is not covered by a legal definition of privilege (such as in Medicine or Law).

  • The Coach agrees not to disclose any information pertaining to the Client without the Client’s written consent. The Coach will not disclose the Client’s name as a reference without the Client’s consent.

  • Confidential information does not include information that: (a) was in the Coach’s possession prior to its being furnished by the Client; (b) is generally known to the public (c) is obtained by the Coach from a third party, without breach of any obligation to the Client; (d) is independently developed by the Coach without use of or reference to the Client’s confidential information; or (e) that the Coach is required by law to disclose.


Cancellation Policy:

  • Client agrees that it is the Client’s responsibility to notify the Coach 24 hours in advance of the scheduled meetings. Sessions cancelled, missed or changed less than 24 hours in advance of any Scheduled Session will be considered as a full Session and charged accordingly.

  • Note: Emergencies happen! If Client misses an appointment due to an emergency circumstance and informs the Coach within 24-48 hours following such circumstance, Coach agrees, in good faith, to waive the above Cancellation Policy.
